Nursing care in the pre-endoscopic treatment of patients with gastrointestinal bleeding due to esophageal varices

Abstract

Introduction: Upper gastrointestinal bleeding is treated by a multidisciplinary team, which includes the nursing professional. The nurse must be specially trained in the management of these patients, to achieve hemodynamic stability  prior to endoscopic therapy, as a guarantee of successful and safe treatment.

Objectives: To describe the nursing procedures in the pre-endoscopic treatment of gastrointestinal bleeding due to esophageal varices.

Methods: A descriptive and retrospective study was performed including information referred to pre-endoscopic care in the bleeding unit of the National Center for Minimal Access Surgery, between January to October 2017. Forty-six cirrhotic patients with esophageal variceal bleeding were included.

Results: Information was collected from the medical records of 46 patients, predominantly male (n=40; 87 %), 23 of them older than 60 years. The main etiology of cirrhosis was alcohol consumption: 38 patients (84 %) of the patients. Nursing actions, including the administration of vasoactive drugs, which made possible the recovery of hemodynamics in 60 % of the patients, before entering the endoscopy room, were listed.

Conclusions: The relevance of the application of the nursing care process in the pre-endoscopic care of the cirrhotic patient with esophageal variceal bleeding is confirmed.
