Liver diseases in pregnant women in a care center for severe maternal morbidity

Abstract

Introduction: There are different clinical conditions that can occur during pregnancy that affect the liver with varying severity.

Objectives: Determine the frequency of liver diseases specific to and coincident with pregnancy, their main clinical and humoral manifestations and the impact on the course of pregnancy.

Methods: Observational, cross-sectional and descriptive study at the Enrique Cabrera hospital in Havana, Cuba, in the period October 2019/December 2021. Including all pregnant women with liver diseases associated or concomitant with pregnancy, treated in a specialized consultation for pregnant women with disease. associated liver disease from the institution's gastroenterology service or admitted to the obstetrics and gynecology service in the indicated period. Information was collected on clinical variables, diagnosis and history of interest. The data were processed with descriptive statistics techniques.

Results: The average age was (mean 26.6; SD 6.52) years. They highlighted a history of high blood pressure (14; 26.4%) and malnutrition or nutritional disorders due to defect (9 with a history of malnutrition, 14 underweight) or excess (17 overweight and 10 obese). 34 of them (64 .1%) had used oral contraceptives. The diagnoses were hyperemesis gravidarum (n=6; 11.31%), preeclampsia/eclampsia (24 patients) and 13 (24.5%) with intrahepatic cholestasis of pregnancy. It was favorable.

Conclusions: The similarity in the clinical presentation of the diseases during pregnancy in the context studied with the international experience was confirmed. Specialized care contributed to the good progress of the pregnancy, with favorable results for both mother and child.
