Expresividad clínica de la enfermedad hepática en pacientes con adicción alcohólica.

Introducción: El consumo excesivo de alcohol es una de las principales causas de enfermedad hepática en Cuba.
Objetivo: Determinar la frecuencia con que aparece la enfermedad hepática alcohólica y su expresividad clínica en un grupo de pacientes con adicción y consumo elevado de alcohol.
Método: Estudio descriptivo, transversal con pacientes alcohólicos atendidos en consulta de Psiquiatría del Hospital Universitario “General Calixto García” entre septiembre 2018 y febrero 2021, que presentaron elementos clínicos que hacían sospechar afectación hepática, por lo que fueron evaluados en el servicio de Gastroenterología de esa institución.
Resultados: Fueron estudiados 99 pacientes, todos con más de 2 puntos en el cuestionario CAGE y con conductas de alto riesgo o dependencia alcohólica. Hubo predominio del sexo masculino (91,9 %), la media de edad fue de 53.1±12.7 años. Se refirió consumo de alcohol por más de 20 años (media 26.7±12.2 años). El nivel escolar más frecuente fue el secundario, (53,5 %). En 77 (77,8 %) se diagnosticó cirrosis, la mayoría (49; 63,6 %) en estadio B de Child. La presentación clínica fue con edemas (63,6 %), ictericia (57,6 %) y ascitis (49,5 %). El laboratorio clínico mostró inversión del índice ALAT/ASAT y aumento de la GGT GGT (mediana 95 U/l). El 62,3 % de los cirróticos tenían várices esofágicas.
Conclusiones: los pacientes con adicción alcohólica presentan en su mayoría una enfermedad hepática avanzada, en estadio de cirrosis y con las complicaciones que de esta se derivan.
